# Chrysalis Recovery Center LLC

## Organization
Chrysalis Recovery Center LLC is located at 112 S. State Street, Concord, New Hampshire 03301. Office hours are Monday to Friday, 8:00 AM to 4:00 PM.
Chrysalis has provided counseling since 2009 and has been an approved IDCMP and aftercare provider since February 2013.
Chrysalis is a State Approved Impaired Driver Service Provider (IDSP). All staff hold the IDSP credential. IDCMP is the Impaired Driver Care Management Program.
Staff are licensed in New Hampshire, Massachusetts, Vermont and Maine, enabling clients in those states to be seen across state lines by telehealth. Most major insurance plans are accepted. Chrysalis holds a contract with the U.S. Department of Veterans Affairs as a VA care provider and accepts TRICARE.

## Fees
IDCMP fees are intake $100, state fee $70, evaluation $275, case management $80 per month, and class $400, or $200 for clients on verified hardship. Case management is monthly and continues for as long as the client is in the program, so the total depends on each client's length of time in the program. [
