Above all, cancers that express excess amounts of the human epidermal growth factor receptor 2 (HER2) protein are now being treated with novel therapeutics. HER2 is a protein on the surface of cancer cells. In some cases, cancer cells produce higher levels of HER2, sometimes referred to as overexpression, and in other cases, the DNA of the cancer cells can have more copies of the HER2 gene, referred to as HER2 amplification. For gastric cancer, in particular, cancers with HER2 overexpression or amplification can grow faster and develop differently than HER2-negative cancers and therefore may benefit from different treatments. Improving HER2 Testing and Diagnosis Accurate assessment of the tumor is key to accurate diagnosis and to decide whether to treat with a HER2-targeted therapy. Cancer tumor(s) can be tested for expression of the HER2 protein and for HER2 gene amplification using laboratory testing of tissue from the tumor(s). This can be done by using methods of immunohistochemistry (IHC) and/or in situ hybridization (ISH). Pathology and laboratory medicine for gastric cancer is becoming increasingly precise, thanks to modern testing methods. Gastric cancer can be characterized into distinct subtypes of tumors, allowing for targeting of specific subtypes for treatments. Sometimes, the cancer tests may need to be repeated because of changes to the tumor. When diagnosing cancer, healthcare professionals need to get an accurate understanding of the tumor. Repeated testing helps to ensure that the best treatment option is given to the patient. The Development of Targeted Therapies Targeted treatments for cancer, like HER2-directed therapies, are designed to attack cancer cells by interfering with specific signals that promote cancer cell growth and survival. These contrast with chemotherapy, which attacks all quickly dividing cells. For example, a HER2-directed antibody is designed to bind to the HER2 protein on the surface of cancer cells. For patients with HER2-positive gastric cancer, treatment choices can depend on several factors including the extent of disease, prior therapy, overall patient health, tumor characteristics and clinical guidance from the medical community and from clinical trials. Many approaches to targeting HER2 in cancer are under investigation. Some are based on antibodies to the HER2 protein (trastuzumab is an example of this) and others combine such an antibody with a chemotherapeutic agent. Their use in gastric cancer, positive for HER2 in excess, is being explored by researchers in various clinical trials. Antibody-Drug Conjugates and Other Emerging Approaches Another therapeutic area that is developing fast is that of antibody-drug conjugates. These typically consist of an antibody attached to a potent drug. The antibody recognizes a specific marker on the surface of cancer cells and the drug is then delivered into the cancer cells. HER2-targeted therapies may be used in combination with chemotherapy, depending on clinical factors and treatment goals. Different combinations are currently being studied to establish which will be the most beneficial for patients. For patients and healthcare teams considering her2 positive stomach cancer treatment, understanding how these emerging approaches are being studied can help provide context when discussing available treatment options. Again, these treatments are under investigation in clinical studies and need to be individualized to a patient’s specific situation. Combining Targeted Therapy with Immunotherapy Cancer treatments are currently being developed which aim to switch on the body’s own immune system to fight cancer. These treatments are often referred to as immunotherapies. There are also many studies combining HER2-directed therapies with chemotherapy and immune checkpoint inhibitors and however, it is well understood that these combinations will have variable levels of efficacy based on several biomarkers from both tumor and blood samples. Biomarkers are used to measure patient characteristics that predict whether patients may benefit from treatment. Such information, in combination with clinical information, is being used more for treatment decisions in oncology. Precision Medicine and Individualized Treatment Precision medicine means that instead of every single patient with cancer being treated by the same method, each patient’s individual circumstances (including the extent of the disease, prior treatment, general state of health and preferences) are taken into account for determining the best possible treatment or for selecting a clinical trial. Molecular profiling of a patient’s cancer can give the treatment team, and the patient, further insight into the biology of the patient’s cancer. Even though there is not a test for every cancer type or stage, using more relevant biomarkers will lead to more individualized treatment for patients with cancer as well as leading to more clinical trials for patients with cancer. While cancer has progressed on prior therapies, a tumor’s molecular profile can predict how it will respond to additional targeted or biomarker-directed therapies. By testing patients for additional biomarkers, they can be given the most appropriate treatment for their individual cancer. Advances in Treatment Monitoring New methods for the monitoring of patients with cancer are also being explored. Typically, these are based on imaging, analytical and clinical assessment. The use of novel methods to monitor tumors, such as blood-based liquid biopsy for detection of tumor-derived material, has also become a focal point in cancer monitoring and management. Ongoing monitoring approaches are being studied for their potential role in treatment management. Many of these technologies are still under investigation to establish the full benefits they may have for patients with cancer but have potential for managing cancer and monitoring for recurrence in a more dynamic manner than currently used methods. Managing Treatment Side Effects However, by far the greater area of cancer health care today is composed of innovative Supportive Care or Prevention and Control programs designed to treat late effects of cancer and its treatments as well as to prevent cancer. Side effects of cancer treatment can affect many parts of the body including the digestive system, skin, blood counts and energy levels. Early detection and appropriate treatment of any side effects is important to ensure that patients on cancer treatments are managed to complete their treatment in good health when that is in the best interest of the patient. Side effects on the digestive system, skin, blood, and energy are common with cancer treatment. There are many ways to deal with late treatment effects. Patients and their families need to be aware of the support available to them and it is very important to have good communication with patients and their oncology team and to contact them straight away if any new or worsening symptoms occur. The Role of Clinical Research Many clinical trials are available to patients with HER2-positive gastric cancer and are looking for participants to test new cancer treatments, their combinations, or dosages, as well as to identify the patients who would benefit most from a certain treatment. While there is potential for large amounts of uncertainty regarding experimental treatments for patients with HER2-positive gastric cancer, participation in a clinical trial for an investigational treatment can be an option for interested patients. The requirements for participation in clinical trials vary for each trial, and there are often risks of treatment that are not yet fully understood by researchers and healthcare professionals. Looking Ahead More options for the treatment of gastric cancer with HER2 overexpression are becoming available. Tests for the diagnosis of gastric cancer, targeted therapy agents, antibody-drug-conjugates and immunotherapies as well as combinations thereof are increasingly being studied for the treatment of patients with HER2-positive gastric cancer. As ever with treatment of gastric cancer though individualization will remain a key component, and much will depend on the circumstances of individual cases. The real challenge of emerging strategies will be to determine their greatest benefit and how to use them in combination in a safe and effective manner. Future research may improve understanding of tumor biology and support development of more individualized treatment approaches.   Disclaimer: The information provided in this article is intended for educational purposes only and should not be interpreted as medical advice, a clinical guideline, or a recommendation for any specific treatment. - [The Case for Culture in Contemporary Dementia Care](https://wellnessvoice.com/the-case-for-culture-in-contemporary-dementia-care/): As the focus on dementia care grows, ensuring that support reflects the patient’s culture, language, and family circumstances is important. The latest workplace data raises an important question: Is the individual always where the problem begins? Gallup’s 2026 workplace research found that nearly a third of full-time employed women in the US report feeling burned out very often or always, compared with 23% of men.
