Sun Health & Dental is NOT health insurance. It is a Discount Medical Plan
Organization licensed and regulated by the Florida Office of Insurance
Regulation under Chapter 636, Part II, Florida Statutes. Members pay
contracted providers directly at the time of service at published fixed
rates. Sun Health & Dental does not pay claims and has no payor ID.

- Individual: $40/month
- Couple: $45/month
- Family (up to 5): $50/month
- One-time application processing fee: $30
- No waiting periods. No pre-existing condition exclusions. Guaranteed acceptance.

## Coverage areas

Primary: Miami-Dade, Broward, and Palm Beach counties, Florida.
Also serving Central Florida and Florida's west coast.

## Services included

Primary care, specialist physicians, dental, vision, laboratory testing,
diagnostic radiology, urgent care, telemedicine (no per-visit cost),
pharmacy discounts, hospital bill negotiation, prescription assistance.
